Purpose
This self-help article will assist end users to change their Name, Date of Birth, or Social Security Number (SSN).
Requirements
The user of this article must have an IDMS Account in order to successfully accomplish the following procedures.
Step by Step Procedures
At times, it is determined that your name, birth date, and or Social Security Number (SSN) information is stored incorrectly in the system and will need to be changed. OSU IT does not have permission to make changes to name, birth date, and or SSN information. Admissions, the Registrar, or Human Resources (HR) must do these changes.
Students
You should contact the Admissions or Registrar office of your campus to request name, birth date, or SSN changes. The information change form for the registrar may be located at: https://registrar.okstate.edu/forms.html. Look for the Correction or Change Of Name link.
Employees
You should contact the HR department of your campus to request name, birth date, or SSN changes. The information change form for Human Resources (HR) may be located at: https://hr.okstate.edu/docfiles. Look for the Personal Information Form (PIF) link.
Verification
Following the steps in this section will verify the steps in the previous section were completed successfully.
Students
- The Registrar must confirm that the requested changes have been provisioned.
