IF YOU HAVE A STATE ISSUED ID:
• Upload one photo of you holding a government-issued ID that contains your picture and name (driver's license, passport, etc.) next to your face in a selfie-style, ensuring that both your face and ID can be seen clearly within this photo and are not blurry.
• Upload one photo of your handwritten name and handwritten signature on a piece of paper.
• Please do not include an ID that contains private information (such as a social security number).

IF YOU ARE A HIGH SCHOOL STUDENT and do not have a state issues ID:
• Upload one photo of you holding your high school ID next to your face in a selfie-style, ensuring that both your face and ID can be seen clearly within this photo and are not blurry.
• Upload one photo of your handwritten name and handwritten signature on a piece of paper.
• Upload one picture of a document from your high school showing your name and the school’s name (report card, transcript, screen shot of online class)

Your Identity Verification Form will not be accepted without the documents above.

To attach multiple photos: 
Select the Browse button within this form, then select multiple photos from your files by holding the shift key while you select (left click with your mouse) both of the photos. If you selected the photos correctly, the files field will say 2 files selected next to Browse. If you are unable to upload multiple at this time, when you receive the ticket in your e-mail, just reply to that with the additional files.
