Overview
This service provides former employees and former students who no longer have access to their IDMS accounts with access to required documents.
Former employees may obtain W-2 forms, and former students may access unofficial transcripts. This is to let the user know that we require documentation of their identity when they fill out the form.
You will need to submit two photos in this form:
- One photo of you holding a government-issued ID that contains your picture and name (student ID, Driver's License, Passport, etc.) next to your face in a selfie-style, ensuring that both your face and ID can be seen clearly within this photo and are not blurry.
- One photo of your handwritten name and handwritten signature on a piece of paper.
- Please do not include an ID that contains private information (such as a Social Security Number).
Your Identity Verification Form will not be accepted without both of these photos included.
Features and Benefits
This service allows end users to request access to their inactive IDMS account for up to two weeks for the purpose of accessing Banner Self Service.
