Used to verify an individuals identity to make changes to an account or recover accounts including bypass codes.

You will need to submit two photos in this form:
  • One photo of you holding a government-issued ID that contains your picture and name (Driver's License, Passport, etc.) next to your face in a selfie-style, ensuring that both your face and ID can be seen clearly within this photo and are not blury.
  • One photo of your handwritten name and handwritten signature on a piece of paper.
  • Please do not include an ID that contains private information (such as a Social Security Number).
Your Identity Verification Form will not be accepted without both of these photos included.

To attach both photos, select the Browse button within this form, then select both photos from your files by holding the shift key while you select (left click with your mouse) both of the photos. If you selected the photos correctly, the files field will say 2 files selected next to Browse.
