Purpose

This self-help article will assist end users to request a professional last name.

Legal name changes should be submitted to Human Resources if you are an employee or your campus Registrar if you are a student.

Valid Reasons for requesting a professional last name are:

• Professional Branding - Individuals in public-facing roles may adopt a name that aligns better with their professional person.
• Marriage or divorce - individuals in public-facing roles wish to retain their professional identity after a legal name change.

The Professional Last Name will be displayed in some systems instead of the legal last name.

Requirements

Step by Step Procedures

1. Go to Service - ID Management Support and click on the 'Professional Last Name Update' button. A new page will open. Fill out and submit the form.
2. Under Subject enter "Request professional last name change"
3. In the description enter your new professional last name. Professional last names should be alphabetic characters only and with no special characters.
4. To provision a professional last name we must request Identity Verification (IV) for data security purposes. Please fill out the Identity Verification in the ticket with the following information.
   • A photo of your signature with your hand written name clearly printed below it (can not be digital)
   • A selfie-style picture of you holding your photo ID next to your face (A&M affiliated ID or a government issued ID in a picture where we can clearly compare your face to the ID photo)
   • Contact phone number
   • Contact e-mail
   • Banner ID (If Known)
   • Date of Birth
   • First and Last Name

5. Once we receive the requested professional last name from you along with the Identity Verification and a picture ID, the requested professional last name will be provisioned. 

Verification

Once the professional last name has changed, you should see the new professional last name in your account.
